I willingly volunteer to care for the kittens/cats for The Catty Shack Cat Rescue and Sanctuary Inc. (CSCRS)**

I am willing to allow a CSCRS Member to perform a home visit prior to receiving a foster. **

I am willing to undergo a background check and will send a copy of my Driver’s License as well as any other adult in my home to CattyShackCR@gmail.com. **

I understand that this cat(s) is temporarily in my care and belongs to CSCRS. **

I understand that the purpose of this foster relationship is to provide socialization and safe and loving care for this cat(s) and that I will be responsible for providing food and litter. **

I understand that this cat(s) must always be kept indoors. **

I understand that if the cat(s) becomes ill, I will notify ANY representative of CSCRS IMMEDIATELY. CSCRS will not be responsible for unauthorized veterinary bills. Symptoms that should be reported immediately include, but are not limited to, any immediate physical injury, failure to eat or drink for more than 12 hours, fever, dehydration symptoms, lethargy, vomiting, diarrhea, hair loss, sneezing, coughing, or any discharge of mucus from eyes, nose or mouth. **

I understand that rescued cats can be unpredictable. I understand that cats in my care may bite, scratch, or otherwise injure, other animals, people or property. I understand and assume full responsibility for any injury which is incurred by any cat that I am responsible for. I specifically indemnify CSCRS, its board and any other member from liability from any cat I am responsible for. **

I agree that CSCRS is not liable to me and/or my children for any injuries, damages, losses, and/or medical expenses incurred to me, any family member, or any visitor to my home in connection with my fostering cats for CSCRS. **

I understand that I may not acquire any additional kittens/cats for fostering without the prior authorization of my CSCRS Representative. CSCRS will not be responsible for medical expenses for any unauthorized kittens/cats or for my personal pets. **

I understand that I have the right to surrender this cat(s) back to CSCRS if circumstances develop where I am unable to give the needed care. **

I understand that when the cat(s) is ready for adoption, I will transport it to an adoption site designated by CSCRS. I also understand that all adoptions will be made through CSCRS using the appropriate adoption procedures, including myself or my family member adopting the cat(s). **

I understand the possibility of disease transmission between CSCRS foster cats and my personal pets and I specially indemnify CSCRS, its board and other members from liability resulting in the transmission of any disease from my assigned foster cat/kitten and my personal pet. **

I understand that regardless of previous history, at NO TIME, will a CSCRS foster cat/kitten be left unattended with a canine of any size or breed. **

I understand that all foster cats must remain separate from other animals in the home until cleared by a CSCRS representative. **

I understand that CSCRS has the right to remove the foster cat without warning for any reason. **

I understand that if I decide to adopt this cat(s), I will pay the current adoption fee. **
